Canadian privacy legislation provides you with the right to obtain access to personal information we hold about you. A request must be submitted in writing to the Aviva Canada Inc.^* (“Aviva”) Privacy Office before we can release any information. This online form will make it easy for you to submit your request.

This form is exclusively for data subjects. Representatives should direct their requests to the legal counsel on file or to the claim's handler for unrepresented claims.

If your request for personal information stems from you having a complaint about services or products offered by Aviva, please first visit the Complaints and Customer Concerns page, which may provide assistance in addressing your concerns.
